Notice of Privacy Practices

How your health information is used, disclosed, and protected, and the rights you have regarding it.

This Notice describes how SensibleDoc, P.C., a Wyoming professional corporation ("SensibleDoc," "we"), uses, discloses, and protects the health information created or received in connection with your care, and the rights you have regarding that information. We protect your health information consistent with applicable Wyoming law and federal consumer-protection law.

1. About this notice

SensibleDoc is a cash-pay practice and does not bill health insurance. We provide this Notice as a matter of good practice and to satisfy applicable state-law privacy obligations.

2. How we use and disclose your health information

For treatment

We use and share your health information to provide and coordinate your care — for example, among your physician, the care team, laboratories, and the pharmacies that prepare and ship your medication.

For payment and operations

We use your information to bill and collect for your membership and medications, and to run the practice and Platform (quality improvement, care coordination, compliance, and support).

With service providers

We share information with vendors that perform services for us (such as cloud hosting, payment processing, AI processing, e-prescribing, and lab interfaces) only under written agreements requiring them to protect it and limit their use of it.

As required or permitted by law

We may use or disclose information when required by law; for public-health and safety activities; to avert a serious threat to health or safety; or for legal process, as permitted by applicable law.

Marketing and sale

We do not sell your health information, and we do not use it for third-party advertising. Uses beyond those described here are made only with your authorization, which you may revoke (except to the extent we have already acted on it).

3. Communications

We may contact you through secure in-app messaging, and by email, text message, or phone, with reminders, treatment information, shipping updates, and other communications related to your care and membership.

4. Your rights regarding your health information

  • Access and obtain a copy of your records, in the form you request when readily producible, for a reasonable cost-based fee.
  • Request a correction or amendment of information you believe is incorrect or incomplete.
  • Request restrictions on certain uses and disclosures, and request confidential communications by alternative means.
  • Withdraw an authorization you previously gave.
  • Be notified if a breach affects the security of your health information, as required by applicable law.

5. How we protect your information

We use administrative, technical, and physical safeguards — including encryption in transit and at rest, access controls, and activity logging — designed to protect your health information. Retention periods are described in our Privacy Policy.

6. Complaints and contact

If you believe your privacy rights have been violated, contact us at support@sensibledoc.com. You may also contact the Wyoming Attorney General’s Office or the U.S. Federal Trade Commission. We will not retaliate against you for filing a complaint.
